2012 High School Registration

Below are all the forms for the 2012 season. Please complete Player Information Sheet, Lincoln Code of Conduct, LHS Girls Lax Release and Waiver, the Blue Med form (need to pick up at Lincoln) and Physical Clearance (unless on file already at Lincoln) and return with a payment of $395.

Return to box in office or mail to Lorrie Shoemaker (address on Registration Info).

Also – you will need to purchase a US Lacrosse membership (uslacrosse.org) for a HS player (cost $35) – this is mandatory to play.
